Referring to Jump Into Action Health & Wellness is simple and secure. We partner with therapists, providers, and community organizations to support individuals who may benefit from psychiatric rehabilitation services.
Referrals can be submitted directly through our referral form. Once received, our team will review the information and follow up to discuss eligibility, next steps, and coordination of care.

Our team will contact the referring provider to coordinate services and discuss enrollment.
